THE COLLABORATIVE FAMILY HEALTH CARE COALITION CONFERENCE "TRANSFORMING THE PRACTICE OF HEALTH CARE: THE COLLABORATIVE SOLUTION" A Conference for Physicians, Family Therapists, Nurses, Nurse Practitioners, Physician Assistants, Social Workers, Psychologists, Hospital and Managed Care Administrators, Policy Makers, and other Health Care Professionals WASHINGTON, DC / JULY 20-23, 1995 Collaborative Family Health Care offers a radical new approach to health care reform. Instead of focusing on who will pay,the collaborative approach focuses on improvements in health care delivery that increase efficiency and effectiveness and cut costs no matter what form of payment is in effect. Collaboration maximizes resources, improves health, emphasizes prevention and transforms the treatment experience for all involved: providers, patients and families. The model rests on three key concepts. First, teams of health and mental health professionals work in concert. Second, the biological, psychological, and social aspects of treatment are given equal importance. Third, the patient's family is included as a crucial component of treatment. At the conference collaborative teams from across the country will demonstrate their working models. They'll answer questions about how to adapt collaboration to fit your work setting and how to market it to your professional community. Equal time will be devoted to skill-based workshops by master teacher/practitioners and to group discussion. You'll meet colleagues from other disciplines, share your expertise and practice innovations, and help to co-create this new field as you attend Workshops, Expert Consults, Poster Sessions, Networking Lunches, Banquets, Book Fairs and Exhibits. You will leave the conference thinking and practicing in new ways and with important connections to colleagues in the collaboration community. These are exciting and challenging times for health care. This is a conference you can't afford to miss.
